We employ knowledgeable and experienced team of Doctor of Physical Therapy (DPT) and Neurologic Certified Specialist (NCS), Licensed Vocational Nurse (LVN), Master of Health Care Administration (MHA) in Long-Term Care, and qualified caregivers that will monitor and provide for your loved ones care.
What is the Carelife Process
- We listen closely and understand your unique concerns and challenges
- We conduct free, no obligation, in-home and fall risk assessment to identify your loved ones condition and needs
- We educate you regarding your care options and costs, and provide helpful community resources
- We develop a care plan together with the family or loved ones based on the findings
- We select the most appropriate caregiver based on the information gathered during the assessment
- We provide an opportunity to meet and interview our caregivers prior to start of care
- We execute the plan
- We perform ongoing quality checks over the phone, as well as performing in-home supervisory visits of our caregivers and re-assessments of your loved ones to ensure their needs are being met, making adjustments to the care plan as needed
